Here's a sure-fire fix: I call it "audio siphoning".
A lot of the phones that use one port for virtually everything (data transfer, charging, audio, phone headsets, etc.) use a simple check - is there a microphone present? If so, allow audio through the port. If not, restrict the port to data/charging only.
The following is a tad kludgy, but it's 100% reliable (I've used it with both the G1 and the MyTouch 3G, on all updates including Donut).
1) Attach the adapter
2) Attach a standard wired headset to the adapter (sound is now routed to adapter & is heard thru the headset)
3) Attach the headphones to the appropriate jack (sound is heard thru headphones as well)
4) Unplug the headset (sound is still heard thru headphones UNLESS or UNTIL you unplug either the adapter from the phone or unplug the headphones from the adapter)
Note: the above may, depending on the setup of the phone, automatically launch the Media Player. If, like me, you want to listen to streaming audio, simply close the Media Player.

so you keep the headset and headphones connected? i dont have a wired headset, so cant try.
