The filesystem security issue could be be a problem, i think though an easy fix would be for the phone to format the sd card with a set partition that is its native filesystem for app storage...

Exactly. Would require installing extra software on a Windoze PC to enable it to read Linux filesystems so apps could be moved from one card to another. M$ still likes to imagine it's the only OS in the known universe. A workaround might be using the FAT filesystem, but with specially encrypted files.
...i'm not sure if you can partition the data on a flash card the same way you can on hard disks...
Oh yeah, a card can be partioned just like a disk. I don't see any obvious benefit to it in this case.

Yes, they can. They handle it poorly if the phone is running. Possibly terribly if an app installed on the SD card is running at the time. IOW, it's not fully perfected to the point of idiot-proof.

That's probably only a minor consideration. A bit of code in paid apps could prevent installation to a SD card, still allowing installation of free stuff to SD. It's really about the sheer technical difficulty of the problem and getting it idiot-proof enough for customer support.
