I think I fixed it. In gmail settings, my sent folder wasn't set to sync. When I set it at 4 days, my outbox cleared.
hey! try turning off autosync then force a sync manually, then turn autosync back on? see if that makes a difference.

This is the fix which I came up with to work around this bug:
1. Open Label Outbox
2. Select original offending message.
3. Scroll down to end of message and select Reply and then Save draft.
4. Close Outbox and open label Drafts.
5. Open email and there is your unsent message which can now be sent, copied or discarded!

Thanks to "gchead" -- his work around ROCKS!!! Works on T-Mobile G1 with firmware 1.6
Problems with Sending - Email stuck in Outbox - GMail stuck in Outbox
1. View Label - Outbox
2. Select original offending message.
3. Scroll down to end of any portion of message thread and select Reply and then Save as draft. NOTE: You cannot reply to the stuck message (there is no button), so just select Reply in any message in that thread.
4. Close Outbox and View Label - Drafts.
5. Open that email and there is your unsent message which can now be sent, copied or discarded!
This was THE only solution that worked. It doesn't matter if you have a photo attachment or not, this solution works. You can also clear the GMail Data and the GMail program, but that deletes the entire email string for anything that was stuck in the Outbox.
If you haven't got a fix for this already, I found that when I turned on the phone with no SIM, turned it off, re-inserted the SIM and powered on - this fixed it.
That was after messing around with Syncing and clearing the program data for every Google app (which didn't seem to do anything).
Hope that helps.

BTW, the fix of saving the offending email as draft didn't work for me.
