Problem With Google Voice and App

I have been using Google Voice and the GV Call Free app for several months and it has worked fine. Last week, my HTC Incredible notified me that an update was available, so I chose to install the update. Since then, it has not worked. It will ask me if I want to make Use GV Callback or Use Normal Call. If I select Use GV Callback, it says it’s Logging In, Placing Call, and then it doesn’t do anything.

I tried uninstalling GV Call Free and installing the newest beta version, and even tried installing an old version. Neither worked.

One other thing I tried was to see if my Google voice account is working by trying to log in through my PC at http://www.google.com/voice/ , and then tried to place a call through the Web interface. It wouldn't work. Any ides or solutions?

Same on T-Mo MyTouch 3G 2009 (OS 1.6) TMO tech said my recently broken "Call with Google Voice" needed the Market UPDATE. Now, Google Voice is set to permanently OFF, can only make GV Call via my computer.
